Abstract
This chapter discusses the long-term outcomes of the first reported series of rectal cancers managed with total mesorectal excision (TME) by a single surgeon (Dr. R. J. Heald). A total of 115 patients were included with 60% having an anastomosis below 5 cm. After a median follow-up of 4.2 years, the 5-year cumulative risk of local recurrence was 3.7%, and the 5-year overall survival was 87%. These results are of particular significance when contextualized in an era where local failure rates were as high as 30%, and no perioperative chemotherapy and/or radiation were administered. This landmark study popularized the implementation of standardized curative TME as the standard for rectal surgery.
