Two column fractures of the acetabulum are complex injuries requiring careful assessment evaluation and planning. Good knowledge of surgical approaches is essential for optimum outcomes. These injuries should be manages in specialised centers with dedicated pelvic and acetabulum surgical teams. In this section, variations on a theme will be presented as there is a massive spectrum of injuries that involve two columns of the acertabulum.
