Abstract

In a small village in Hokkaido, we examined a community‒based comprehensive geriatric assessment study included the ToCA‒MCI. Using cut‒off points of 17/18, we can predict the progression from MCI to dementia in 121 elderly Japanese(81 women, 65 to 95 years of age)followed‒up for 6 years. Kaplan‒Meier analysis showed that the frequency of occurrence of dementia was statistically significantly higher in the 40 citizens who had a ToCA‒MCI score 17 or less(Group B)compared with the 81 citizens with had a ToCA‒MCI score 18 or more(Group A)(Cox‒Mantel test, p=0.0038). As life‒style intervention, a 2‒hour exercise class was offered once a week for 12 months to 14 of the 40 citizens of Group B(Intervention Group). They could walk faster(p=0.0001)and were less likely to become frail(p=0.0077)than the other 26 citizens of Group B(Control Group). Kaplan‒Meier analysis showed that the frequency of occurrence of dementia was statistically significantly lower among the 14 citizens in the Intervention Group compared to the 26 citizens of Control Group(Cox‒Mantel test, p=0.0105). Lifestyle interventions that help improve the walking speed and prevent frailty are therapeutic targets to delay or prevent the onset of dementia in community‒dwelling citizens.
